When I first started purchasing in 2020, I assumed that if a device was FDA cleared and the sales rep sounded confident, my job was to negotiate the price. Three budget overruns later, I realized the real job begins before the demo. I now use one checklist for every device decision: ordering Holter monitors for cardiology, replacing surgical staplers for the OR, or evaluating BTL equipment with the aesthetic and gynecology teams.
A quick warning before the steps: if you search for "BTL" online, you'll get finance articles about buy-to-let mortgage rates, because BTL means something different in banking. Ignore that noise. The BTL I work with is the medical device manufacturer—the company known for Emsculpt, Exilis, and a BTL gynecology product line. Knowing the difference matters more than you'd think.

This is for the buyer who isn't a clinician: office managers, clinic administrators, operations leads, and procurement people. You don't perform the procedures, but you approve the purchase. You're responsible for compliance, total cost, and making sure the staff actually uses what you buy. This checklist gives you a repeatable process for all of it.

Step 1: Write Down the Intended Clinical Use Before Comparing Features
This step sounds too basic to include. It is basic, and it's the step people skip most. "We need a BTL device" is not a purchasing requirement. "We need to evaluate the BTL gynecology line for pelvic floor therapy" is a real project. "We need a body-contouring system" is a different project. If you don't know which one you're buying, you'll compare machines that were never meant to be compared.
Ask the requesting clinician to describe the patient population, the condition being treated, and the expected workflow. For example, a Holter monitor request doesn't tell you enough. Do you need 24-hour ambulatory ECG recording or 14-day event monitoring? Does the data need to stream to the EHR in real time, or can the patient return the device at the end? These details change the device class, the price, and the vendor shortlist.
Seriously, that one question filters more bad fits than any other step in this process.
Step 2: Verify the Regulatory Status Yourself
Don't rely on the pitch deck. If you're in the US, look up the device in the FDA 510(k) database at accessdata.fda.gov and read the exact indication language. The difference between "cleared for X" and "cleared for Y" can be significant. If you're buying in Europe or the UK, ask for the CE certificate or MDR documentation and check that the legal manufacturer name matches the vendor.
A common misunderstanding I see everywhere: "FDA cleared" doesn't mean "FDA approved" or "clinically proven." It means the device is substantially equivalent to a legally marketed predicate. That's an important distinction, especially for surgical tools like staplers or energy devices where the marketing claims always sound broad.
If a sales rep can't give you a regulatory number or a copy of the clearance letter, that's a red flag. If they can, it makes your job easier. When our team evaluated BTL equipment, the regulatory documentation was available up front, which is how it should be.
Step 3: Ask for Clinical Evidence, Not Adjectives
You will see three kinds of proof in medical device sales: brochure phrases, patient testimonials, and published clinical data. Only one of those should influence your decision.
Ask the vendor: "What peer-reviewed studies have been done with this device in a population similar to ours?" Then check it yourself on PubMed or ClinicalTrials.gov. If the vendor can't point to anything, that's a serious problem. Per FTC advertising guidance (ftc.gov/business-guidance/advertising-marketing), marketing claims have to be truthful and substantiated. A manufacturer should be able to back up what they're telling you.
When our group looked at BTL aesthetic and gynecology products, the reps brought published studies and were open about which patient groups had been studied. That's the standard I now expect from every vendor.
This step applies even to seemingly simple purchases. When we evaluated surgical staplers, I couldn't evaluate "feel" the way a surgeon could. But I could ask for data on hemostasis, leak rates, and the range of reload sizes. If a vendor can't produce comparative data, they don't get the order.
Step 4: Price the Total Cost of Ownership, Not the Sticker Price
This is where an administrative buyer can actually outperform everyone else in the room. The capital cost is only one line item. You also need to price:
- Installation, freight, and first-year training
- Consumables and single-use components
- Annual service contracts and preventive maintenance
- Software licenses, data integration, and network setup
- Loaner availability during repairs
A surgical stapler is a classic example. The initial handle might be priced competitively, but the reloads are single-use. Depending on case volume, the reload cost can exceed the hardware cost in the first year. Ask the vendor to calculate the cost per case, not just the cost per device.
Holter monitors taught me this lesson the hard way. In 2024, I approved an order based on the recorder price, but the analysis software and network gateway were quoted separately. The unplanned setup cost added roughly $1,800 to the project. Note to self: always ask for the complete data workflow cost in the first RFQ.
For BTL equipment, the same logic applies. Ask about applicators, single-use items, service schedules, and what happens if a treatment head fails. "Cheap" in medical devices usually means someone else pays later.
Step 5: Understand How the Product Is Actually Used and What Support Comes With It
You don't need to be a clinician, but you need to understand the basics of how the technology works. Otherwise, you can't evaluate whether the training plan is adequate.
Take robotic surgery. When our surgical team first asked me to help evaluate a robotic platform, I had to learn how robotic surgery works before I could ask intelligent questions. The system has three main parts: a surgeon console, a patient cart with robotic arms, and a vision system. The surgeon sits at the console and controls instruments with articulating tips that mimic hand movements inside the patient. The machine doesn't operate itself; it translates the surgeon's movements with more precision and range of motion than the human hand allows. Once I understood that, I understood why training and service support matter more than the hardware price.
Ask the vendor specific questions:
- How many training sessions are included?
- Who does the training, and where does it happen?
- Is there a clinical specialist available during the first several real cases?
- What is the service response time if the device goes down?
- What happens if key staff leave and new staff need training?
When I first started buying equipment, I treated training as a nice extra. It isn't. It's the difference between a device that improves patient care and a very expensive piece of furniture.
Step 6: Collect Stakeholder Sign-Off Before You Sign Anything
This step kept me out of trouble more than once. Before issuing a purchase order, I send the proposal to four people:
- Biomedical engineering—can they service it internally?
- IT—does it connect to our network securely and integrate with the EHR?
- Compliance or quality—is the regulatory documentation complete?
- Finance—does the depreciation schedule and CPT billing code make sense?
Clinicians need to sign off too. If the device is bought without clinical user input, don't be surprised when it sits unused in a storage room.
If a vendor hesitates to provide documentation for these stakeholders, consider that a serious warning. The right vendors understand that procurement is a team sport.
Common Mistakes I Still See
- Searching the brand name online and getting confused by unrelated finance content. Use official sources and the company's own regulatory documentation.
- Treating "FDA cleared" as proof of clinical effectiveness. It isn't.
- Comparing only capital costs and ignoring consumables, software, and service.
- Assuming training is included and comprehensive. Verify it in writing.
- Buying the newest model with the most features when the staff doesn't have the skills to use them.
Bottom Line
Five years ago, a lot of procurement decisions came down to price and the sales rep's confidence. The fundamentals haven't changed—regulatory status, clinical evidence, real support, and total cost still matter. But the execution has changed. We now have access to FDA databases, published clinical research, and vendor documentation in ways we didn't have in 2020. If you're not using those tools, you're making the job harder than it needs to be.
The same checklist applies whether you're buying a Holter monitor, a surgical stapler, a robotic surgery system, or BTL devices across aesthetics and gynecology. Do the verification, get the evidence, involve the stakeholders, and you'll be fine.
